Use that for the action attribute, and it should work. It is copied straight from the cobalt theme, except I have appended &theme=Originals to the end.

You don't need the '&theme=Originals' bit at the end. The bug was the 'item_id={$PHOTO_ID}' which as Kyle corrected should have been 'photo_id={$PHOTO_ID}'.

I think I fixed this some time ago, and has certainly been done in the latest version of the theme currently available.
