I've just done this with the following themes -

Apple
Centrifuge
BloodyRoom
KL Clouds
Dun Forest
Sandstorm
Moss Green
Cobalt

Just add the following bit of code to the 'song_header.tpl in each of the theme folders -

&nbsp;&nbsp;<i><a href="{SONG_RATING_FORM}" {POPUP_JAVALINK}>Rate This Song!</a></i>

I put it after the {SONG_NAME} macro as follows -

<tr>     <td class="thMusicH">&quot;{SONG_NAME}&quot;&nbsp;&nbsp;<i><a href="{SONG_RATING_FORM}" {POPUP_JAVALINK}>Rate This Song!</a></i></td>     <td class="thMusicH"><span style="text-align: right">{SONG_LENGTH}</span></td>   </tr>

All seems to work OK

EDIT
Just found out that if you change {SONG_RATING_FORM} to {BAND_RATING_FORM}, you can use the same code to rate the Bands as well. Going to see where I can fit that into the themes
