It looks like you're using a custom module - I would recommend _not_ using getMediaFile, but use jrMediaGetFile instead:

This actually has happened to me as well but i am not sure whats up what it will do is if you create a vault entry it will put the mp3 in 1 location (make 2 entries) and then you have to delete the other entry and edit the other entry and that fixes the issue, but not sure what caused this issue.
