The top field it the links URL the second field is the text you want to make a link.
Are you saying that you can't put text in the second field?

Make sure the profile your adding the HTML code too is allowed all HTML tags.
Core > System Core > Quota Config
Be sure to select the quota your trying to add the code to and the Allowed All HMTL box is checked.
Did you run the Integrity Check tool?
ACP > System Tools > Integrity Check
You may also need to reset your cache...

How do you customize the tiny MCE editor buttons?

You can modify the editor menu (add/remove buttons) in your modules/jrCore/templates/form_editor.tpl
If you don't want image uploads, you can disable it per quota in your ACP > Forms > Editor Image Upload > Quota Config

If you have that form setup for logged in users only, it will also not show.
