Use the 'Simple Custom Form' module and enable the default 'Contact Us' form that comes with it. When this is done Flashback will detect that that form is in place and link to it in the footer template.

...alternatively, just hit the 'Delete' button on the index page SiteBuilder menu (sorry, forgot about that simpler method!!).

You have enabled SiteBuilder for the index page. To get back to the original skin front page goto the ACP=>Site=>Site Builder=>Tools=>Page Browser and delete the index 'site builder' page (the one with the '/' url).

Difficult to tell from your screenshots but there looks to be iframe tags in the code. Iframes are a security risk when used on sites and Jamroom strips them out by default. If you need to use them, install and configure our Iframe Control module - https://www.jamroom.net/the-jamroom-network/networkmarket/116/iframe-control

Maybe point me to the player code then if its on another site somewhere?

Enable Keep-Alive

'Keep Alive' is a server function. Jamroom has nothing to do with keep alives.
