simple custom forms form partially showing

I have the contact-us form created by default with the simple custom forms module. It's only showing the Human Check form element when not logged. I don't have the "Logged In" option checked. I'm sure it was working at one point.

I remember something about this from back in beta. I think it was that it isn't possible to complete the signup and login without being human, so the human check isn't relevant for a logged in user.

Actually, when I'm not logged in it's ONLY showing the human check. The rest of the form doesn't show. Which defeats the purpose of general purpose contact form.

No. It looks like the variables are set so it should show the form. Hopefully someone who built the module can find a ready solution.

I'm not see this issue here - when viewing the contact_us form, click on the form designer button and make sure you've not restricted the fields to specific quotas or user groups.

They're all set to (group) normal user, which I think should be OK.

I tried setting one of the fields to all of the selections, but it's still not showing.

Is the "Form Login" option checked in Modify for the form? I really don't know what this would be as I'm not able to duplicate it, and it's a really simple setup.

No, Form Login is not checked. The database confirms this value.

I made a new form, but that one doesn't work either. Behaves the same. When not logged in, only the Human Check shows.

Yeah something is screwed up in the setup on your system - I don't see any problems with it here.

I would have to have FTP access and login access to your site to see what's up (and based on your other post about privacy, I'm guessing there is a 3rd party module, or other customization that could be messing things up).

I've just fixed a problem with another module by disabling ujTogether. Give that a try first if you have it installed.

I'll look at it tomorrow.

Looks like Together is causing a problem on my site if other modules are using the javascript_footer_function. So try disabling ujTogether if you are using it.

I don't doubt it's something I've done, but the site I'm encountering these issues on is only using jr Modules and I've customized the index.tpl and profile_index.tpl page, but there isn't any logic that would override anything.

Brian, it would be great if you want to take a look. Where do I send the access credentials?
