You need to create the "contact_us" contact form in the Simple Custom Forms module.

You'll want to update your links to the contact us page, and your contact_us.tpl as well. Check the latest version of jrNova and you'll see in the header and footer.tpl files, the links have been modified to work with the simple custom forms module, if it is enabled or not.

What does your contact us link look like in your footer.tpl file?
